We provide direct Geriatric care management services to clients living in Southeastern Wisconsin (Kenosha, Milwaukee, Ozaukee, Racine, Waukesha and Washington counties) area.

Every Senior Life Care Plan includes a private, Geriatric Care Manager who oversees the coordination of your loved one's health and long-term care, serves as your advocate and assists you with the decisions you'll make regarding your loved one.

Whether your loved one is living at home, at an assisted living facility or in a skilled nursing facility, you can relax in the knowledge that his or her care is being coordinated by experts who have extensive knowledge about the costs, quality, and availability of resources in the community.

What is a geriatric care manager?

  • - Certified social worker or registered nurse  with specialized knowledge and expertise in senior care issues.
  • - Somebody who will coordinate and manage all care resources you could possibly need.
  • - Someone who can acknowledge family limitations.
  • - A third party to provide crisis intervention.

What Does a Geriatric Care Manager do?

Your private, geriatric care manager serves as your support line, your confidante and your friend. She will stand by you every step of the way as your advocate, expert and counsel.

  • - Prepare and guide you and your family through the Medicaid application process
  • - Working in collaboration with other members of the support team serving your loved one, including long-term care administrators, accountants, physicians and financial planners to insure that the right professional is helping you at the right time.
